In this section, State Records of South Australia has supplied a number of standard forms for seeking access to documents under the Freedom of Information Act, 1991.
These forms enable you to:
request any document in the possession of a State Government agency, Local Council or University
request the amendment of documents that relate to you which are incomplete, incorrect, out-of-date or misleading
seek a review of a decision made by a State Government agency, Local Council or University.
You will need to print and send the completed application to the agency whom you believe holds the documents to which you are seeking access.
This form is used to make a request for access to any document held by a State Government agency, Local Council or University.
Please print and send your completed application with the application fee, if applicable, to the agency that holds the documents that you wish to access.

Application for access to Cabinet documents
This form is used to make a request for access to Cabinet documents between 10 and 20 years old.
You may either print and send your completed paper-based application to the Department of the Premier and Cabinet via the contact information on the form, or you may lodge the electronic copy via email. For further information about accessing Cabinet documents, please select How do I access Cabinet documents.
This form is used to make an application to amend records about you, if you believe they are incomplete, incorrect, out-of-date or misleading. There is no fee to make an application for amendment.
Please print and send your completed application to the agency that holds the records you wish to amend.
This form is used if you are dissatisfied or concerned with a decision made by a State Government agency, Local Council or University in relation to an FOI application, and you wish to request that an internal review be conducted. An application for review must be lodged within 30 (calendar) days of the previous determination.
Please print and send your completed application to the agency that made the decision.
